HRS §338-5

Who must file a birth certificate and when

A birth certificate must be filled out and filed with the local health department agent in the district where the birth happened, within the time the health department sets. The person in charge of the birth (like a doctor, midwife, or hospital official) must do it, or a parent if no one else was there. The birth facility must also share medical records so the department can check compliance.
