HRS §40-6
Who approves state business and accounting forms
The comptroller decides what forms the state needs for accounting and statistics. Department heads must get the comptroller's approval before printing new or changed forms. The University of Hawaii and the Department of Education only follow this for statewide uniform forms. All standard forms are cataloged and standardized.
