Forms the state can require for business filings

The state business office can create and give out forms for certain filings, like getting a certificate of good standing or a foreign company's permission to do business here. If the office says so, you must use those forms. For other filings, using the state's forms is optional.

(a) The department director may prescribe and furnish on request forms for: (1) An application for a certificate of good standing; (2) A foreign corporation's application for a certificate of authority to transact business in this State; (3) A foreign corporation's application for a certificate of withdrawal; and (4) The annual report. If the department director so requires, use of these forms is mandatory. (b) The department director may prescribe and furnish on request forms for other documents required or permitted to be filed by this chapter but their use is not mandatory.
