Filing proof of workers' compensation insurance

If you are an employer who got insurance under the specified part of the law, you must file a notice about that insurance with the director, using the director's required form, and include a statement of the benefits the policy provides. The director can also accept this notice from approved third-party agencies.

If the insurance so effected is under section 386-121(a)(1), the employer shall file with the director in a form prescribed by the director a notice of the employer's insurance together with a statement of benefits provided by the policy of insurance. The director may also accept the notice of employer's insurance from approved third party agencies in a manner and form approved by the director.
