(a) Every insurance producer doing business in this State shall notify the commissioner in writing of the insurance producer's legal name. (b) An insurance producer shall apply to the department of commerce and consumer affairs and the commissioner for approval of the use or change of a trade name pursuant to section 431:2-217. (c) An insurance producer doing business under any assumed name, other than the producer's legal name, shall notify the commissioner in a form prescribed by the commissioner.
