Tell the board if you move out of Hawaii
If you have a chiropractic license and move out of Hawaii, you must tell the board in writing right away. The board will note it. If you don't, you lose your license and must pay a fee to get it back.
The statute, as written — Change of residence
Every holder of a license who leaves to reside outside the State, shall immediately notify the Hawaii board of chiropractic of the change in writing and the change shall be noted in the board records. Failure to do so constitutes a violation and results in a forfeiture of the license, and it shall not be restored except upon the written application therefor and a payment to the board of a penalty fee.
