Tell the program about name, address, or phone changes

If you are in the address confidentiality program, you must tell the program when your legal name, home address, or phone number changes. You have a short time to send proof of a name change or notice of an address or phone change. This section only covers what you must report and when.

The statute, as written — Change of name, address, or telephone number

(a) Within thirty days of a legal name change, a program participant shall provide the program with a certified copy of a judgment, order, or any other documentation the program director deems to be sufficient evidence of the name change. (b) Within ten days of a change in actual address or telephone number, a program participant shall notify the program of the change.
