A. To the extent permitted by law, the mayor must submit the following information to the council when nominating an individual to a board, commission, or committee under section 13-2, Revised Charter of the County of Maui (1983), as amended: 1. Name of the nominee. 2. Board, commission, or committee, and the specific seat to which the individual is being nominated. 3. Place of residence of the nominee. 4. Mailing address of the nominee. 5. Telephone number of the nominee. 6. Summary of the nominee's previous service on any boards or commissions. 7. Summary of the nominee's qualifications, including any special qualifications established by the Code or the Charter. 8. Any other relevant background information. 9. Statement of consent to the nomination and certification of truthfulness and accuracy of information contained in the application signed by the nominee. B. Information in which an individual has a significant privacy interest as provided by law must not be transmitted to the council on the same form as information available to the public. ( Ord.
