Applicant’s duty to update contact information

If you are on a county waiting list, you must tell the county in writing if your mailing address or phone number changes. If you don’t, and the county can’t reach you, your name will be taken off the waiting list.

Each applicant shall be responsible to notify the department in writing of any changes in mailing address and telephone number. If an applicant fails to properly notify the department of such changes, and the department is unable to contact the applicant by mail or telephone, the department shall remove the applicant's name from the applicable waiting list(s).
