Each applicant shall be responsible to notify the department in writing of any changes
in mailing address and telephone number. If an applicant fails is to properly notify
the department of such changes, and the department is unable to contact the applicant
by mail or telephone, the department shall remove the applicant's name from the applicable
waiting list(s).
