A.
Written application for a dog license shall be made to the licensing authority and
shall include the name, breed, color, age, and sex of the dog. If any dog is certified
as a guide, signal/hearing or service dog, such certification shall be indicated on
the written application.
B.
License fees shall be charged for guide, signal/hearing and service dogs.
